Passport Home Care

The program creates a care plan that mixes different services to help you stay home safely. Services include help with medicines, meals, cleaning, transportation, medical equipment, and changes to your home. A social worker manages your care plan.

You must be 60 or older, live in Cuyahoga, Geauga, Lake, Lorain, or Medina County, and live in a home or apartment. Your income and savings must be low enough to qualify for Medicaid Long Term Care. You must need help with daily activities like bathing, dressing, or meals to stay home.

What to expect
Call Monday through Friday, 8am to 5pm, for a free assessment. A social worker will ask questions about what you need. If you qualify, you'll get a care plan. Some people pay a small monthly fee, but most get services for free.
